Copley Library Interlibrary Loan: Interlibrary Loan Blank Request Form

This guide provides instructions for requesting resources from other libraries through Copley Library's interlibrary loan service.

What Can You Request from Other Libraries?

Copley Library can request print books, book chapters, media, and journal articles for you from other libraries.  The interlibrary loan blank form will prompt you for citation information depending on the type of material you want to request. 

To access a blank interlibrary loan form for Copley Library to fulfill, click here.  You will be prompted to enter your USDOne login and then will have access to the Interlibrary Loan form to fill out with information about your request. 

Interlibrary Loan Blank Book or Book Chapter Request Form

For print book requests, select Book as your option and note that required fields have a red asterisk * in front of them.

If you just need a set of pages or a chapter, you can checkmark the box to alert the library.  If the library can fulfill this request, you will receive the pages or chapter as a digital copy through your library account.

If you are requesting media (DVD, etc.), please add your preferred media format in the Comment section at the bottom of the form.

After filling out the form, click the Send Request link at the bottom.  You will be notified by the library when the book arrives for you to checkout. 

Interlibrary Loan Blank Article Request Form

For journal article requests, select Article as your option and note that required fields have a red asterisk in front of them.

After filling out the form, click the Send Request link at the bottom.

You will receive the article as a digital copy through your library account.
